Viking offers discounted lab assessments for therapy candidacy and health optimization.

Please note the following regarding lab work:

Current blood work within the last 6 months is required.

If your lab results are older than 6 months, consider these options:

• Lab work does not have to be purchased through Viking.
• You can use lab work from your primary care physician, different lab facilities, or other providers within the last 6 months.
• "At home Blood testing kits" are not accepted by the medical staff at Viking.

This is an order form for a Lab Requisition for Blood Analysis, necessary for assessing your physical condition and eligibility for Hormone Replacement Therapy. It also helps in prescribing the appropriate therapy protocol. No payment is required currently. To order, indicate your desired panel by entering "1" in the quantity box and submit the form.

For direct insurance billing instead of a prepaid requisition from Viking, please choose "I would like to submit to my own Health Insurance", below. We will ensure all necessary information is included.
